PrideStaff employment benefits

What health plans are available?
PrideStaff offers a comprehensive medical plan that meets the Affordable Care Act coverage requirements. Voluntary benefits plans such as a Dental Plan, Vision Plan, Hospital Plan and Medical Indemnity are also available.
How do I enroll in a PrideStaff Benefit Plan?
Two to three weeks after receiving your first paycheck, you can enroll by visiting: powerofready.com/ees/pridestaff.html Information on plans and prices are provided on this site. You may also contact the Customer Service at 866.770.6399 if you need any assistance.
Can I enroll whenever I want to?
No. You must enroll within 60 days of your first work day. However, the enrollment site will not be available to you until approximately two to three weeks after you receive your first paycheck. Your insurance will be effective on the first of the month following a 60-day waiting period. You may also enroll during Open Enrollment if you have previously met the 60-day waiting period. You may also enroll within 30 days of a Qualifying Life Event (QLE).
What is Open Enrollment?
Open Enrollment is a period when an employee who has met the 60-day waiting period can enroll in health plans or make changes to one’s insurance coverage with no restrictions. Open enrollment is held in the last quarter of each year, typically November through December.
What is a Qualifying Life Event?
A Qualifying Life Event (QLE) is defined as a change in your status due to one of the following: 1) marriage or divorce 2) birth or adoption of a child or children 3) loss of insurance coverage involuntarily 4) death of an immediate family member 5) Medicare entitlement 6) employer bankruptcy 7) loss of dependent status 8) loss of prior coverage.
Do I need to provide documentation of a Qualifying Life Event?
Yes. A QLE form must be filled out and proof must be provided within 30 days of the event.

If my assignment ends, will I lose my health coverage?
Not necessarily. If you go off assignment you will need to contact the Benefits Department as soon as possible, requesting to pay the premium to maintain your health coverage, otherwise you will be offered COBRA coverage.
What is COBRA coverage?
The Consolidated Omnibus Budget Reconciliation Act (COBRA) provides field associates who lose their health benefits the right to continue their coverage that would otherwise be terminated upon job loss.
Is COBRA coverage more expensive?
If you qualified for an employer contribution, once enrolled through COBRA, there would no longer be an employer contribution and you would be responsible for the total premium. Additionally, there is a 2% administrative fee added to the premium.
How long can I be on COBRA?
Depending on the state you live in and your circumstances, COBRA benefits may extend from 18 to 36 months.
What is the purpose of the New Health Insurance Marketplace Coverage Options and Your Health Coverage notice that was provided to me at hire?
As a result of the new health care reform law, Part A of this notice provides you with information on how you can purchase health insurance through the Health Insurance Marketplace. Part B provides PrideStaff’s information as your employer. Go to healthcare.gov to examine your health insurance options available outside of PrideStaff.
